Visiting us for the first time

We strongly encourage new patients to schedule a visit before an illness occurs.  This gives your new physician an opportunity to learn you or your child's medical history, and gives you the opportunity to learn more about our clinic.  Please arrive 15 minutes before your scheduled appointment to complete necessary paperwork and complete the check-in process.  Please consider downloading the new patient paperwork on the patient forms page to help speed this process up.  If you have a complex medical history, please tell our schedulers about this.  We'd like to assure adequate time is scheduled and sometimes a longer appointment slot would be appropriate.

Bring the following items to your visit

  • A complete list of medication you or your child is taking, or (preferably) the actual medication bottles.
  • Any vitamins, herbal supplements or over-the-counter medication you or our child is taking, since these can affect the body and its reaction to medication we may prescribe.
  • Immunization records and a copy of you or your child's previous records if you are transferring to us from another office.
  • Your insurance card.  To comply with insurance company requirement, we must see insurance cards at each and every visit even if nothing has changed.
